Case Study: A 7-Year-Old Learns to Master Anxiety
How Child-Centered CBT Provides the Tools to Cope
When a young child is struggling, it can be heartbreaking for parents. Anxiety in children doesn’t always look like quiet worrying. It can often manifest as behavioral problems, sudden outbursts, or a strong resistance to new situations.
“Bobby,” a 7-year-old, was referred to MindWell for treatment of his anxiety. His successful journey illustrates how a structured, evidence-based, and compassionate approach can empower a young child with the skills to understand and manage his own emotions.
The First Step: A Clear Diagnosis and a Family Plan
The process began with an initial 90-minute intake session involving Bobby and his parents. This foundational meeting allows our clinician to gather a complete developmental history and, just as importantly, build rapport in a warm, supportive environment.
Bobby was diagnosed with an anxiety disorder. A clear and accurate diagnosis is the essential first step for any effective treatment.
Our clinicians also use this time to determine if further testing is needed. It’s crucial to understand the whole child, as anxiety can often be the first visible sign of an underlying learning disability or attention issue. A key part of the diagnostic process was ruling out other conditions, like depression or ADHD, that can sometimes mimic the symptoms of anxiety.
Once the diagnosis was clear, the plan involved the entire family:
- For the Parents: The clinician explained the diagnosis, providing them with a clear understanding of their son’s struggles and a concrete, proven plan for moving forward.
- For Bobby: The diagnosis was also explained to him using age-appropriate language a 7-year-old could grasp. This is not about applying a label; it’s about empowerment. It helped Bobby find relief by understanding his own feelings and gave him the words to start advocating for himself.
The Treatment: Building Skills with Cognitive Behavioral Therapy
Bobby began a treatment protocol using Cognitive Behavioral Therapy (CBT), which was tailored to his specific age and developmental level. CBT is a highly effective, evidence-based treatment that teaches children to recognize the connection between their thoughts, feelings, and behaviors.
For Bobby, this treatment focused on teaching him to recognize how his body and mind responded to fear and stress. He learned to identify:
- Antecedents: The triggers or situations that made him feel nervous.
- Behaviors: How his body and mind reacted (e.g., fast heartbeat, "worry thoughts").
- Consequences: The skills he could use to calm himself down and take control.
Through a combination of talking, behavioral charts, and structured play therapy, Bobby learned how to anticipate and “read” the anxiety in his body. He was equipped with skills to examine his thoughts and calm both his mind and body, allowing him to adapt to almost any situation.
The Outcome: A Confident Child with Skills for Life
After approximately 20 weeks, Bobby had learned to master these new coping strategies. He was then transitioned to “booster sessions” as needed, allowing him and his family to check in and reinforce his new skills over time.
Bobby’s story is a perfect example of our core philosophy: to get beyond the root of the problem and find the root of the solution. He didn’t just get relief from his symptoms; he gained a set of skills and a new sense of confidence that will help him self-soothe and navigate life’s challenges for years to come.
